This questionnaire collects the information needed to prepare your Petition to Determine Parental Relationship (parentage) along with your custody, parenting plan, and child support documents. Complete every section that applies to your situation — if you're unsure about an item, leave it blank and JoHanna will discuss it with you.

Case Information

Parentage

Parentage (paternity) is the legal determination of the child's parents. For unmarried parents, this must be established before the court can order custody or support.

💡 If parentage is already established, a new parentage case may not be needed — JoHanna will confirm at your appointment.
💡 If parentage is disputed, genetic (DNA) testing may be requested through the court. Note any details below and JoHanna will discuss it with you.

Children
💡 Required for the UCCJEA Declaration — a mandatory court form in every custody matter.
⚠️ Please provide the court name, state, and case number — other proceedings may affect jurisdiction.

Custody Arrangement
💡 Legal custody = who makes major decisions (education, healthcare, welfare) — not where the child lives.
💡 The two percentages should add up to 100%. Timeshare directly affects the child support calculation.

Regular Parenting Schedule

Describe the regular weekly schedule during the school year — which days the children are with each parent.

Holiday Schedule

Which parent has the children for each holiday. "Alternate" means it rotates each year. Leave blank if the regular schedule applies.
