This questionnaire is for clients who have completed the NewLeaf Service Request Form and received a proposal. Submitting it lets JoHanna know you would like to move forward with your proposal. She will then send your Service Agreement and Notice to Consumer to review and sign. No documents are prepared until your Service Agreement is signed. If you have not yet completed the Service Request Form, please start there first.
Please complete every section as thoroughly as possible. If you are unsure of an answer β your best estimate is fine and we will confirm everything together at your appointment. This questionnaire is confidential.

Your petition states what you are requesting. Nothing here is final β it can change as your case goes on. Choose "To be decided" for anything you and your spouse have not settled.
A joint petition is for people who agree, or expect to agree, on everything. You do not have to have it all worked out today — but you do have to get there before a judgment. Where are the two of you right now on each item?
1. Neither of you can ask the judge for temporary orders. Not for support, not for custody, not for the house. The court's own words: “You can't ask for temporary orders with a joint petition.” Any orders come at the end, from your agreement. If either of you needs something ordered along the way, this is not the right process — and asking for one ends the joint process.
2. You have to agree on everything by the end. If you cannot, one of you revokes the joint petition and the case converts to a regular case. That is a normal, expected thing — not a failure — but it adds steps and cost.
3. Either of you can end the joint process at any time before judgment, without the other agreeing.
