This questionnaire collects the information needed to prepare your Custody and Parenting Plan. Both parents must agree on all terms — if you are unsure about any item, leave it blank and JoHanna will discuss it with you at your appointment.

Case Information
⚠️ Please upload a copy of the existing custody order using the upload field at the end of this form. JoHanna will need to review it before preparing the modification.

Children
💡 This is required for the UCCJEA Declaration — a mandatory court form on all custody matters. If all children share the same history one entry is fine.
⚠️ Please provide the court name, state, and case number — proceedings in another state may affect jurisdiction over your children.

Custody Arrangement
💡 Legal custody refers to who makes major decisions about the children's education, healthcare, and welfare — not where they live.

Regular Parenting Schedule

Describe the regular weekly schedule — which days children are with each parent during the school year.

Holiday Schedule

Specify which parent has the children for each holiday. Holidays generally override the regular schedule. "Alternate" means the holiday rotates between parents each year — specify who gets it in even years and who gets it in odd years.

Summer & Vacation Schedule
💡 Standard provision: if children are taken out of California for more than 24 hours, the traveling parent must provide prior notice including dates, destination, and contact numbers.
