Request for Order (FL-300) Questionnaire — NewLeaf Document Services

Request for Order (FL-300) Questionnaire

NewLeaf Document Services · JoHanna C. Cody, LDA #23-003 · Placer County

You've directed me to prepare a Request for Order for your case. Your answers here are the facts I'll use to prepare your court forms at your direction. Only the sections that apply to your request will appear. Your progress saves automatically on this device — you can come back and finish anytime.

NON-ATTORNEY DISCLOSURE: I am not a lawyer. I cannot provide legal advice, and I cannot recommend what to ask the court for or how to ask for it. NewLeaf Document Services is not a law firm. I can only provide self-help services at your specific direction. This questionnaire gathers facts only. For legal advice, please consult a licensed California attorney.

1. Case Information

This affects how your papers must be delivered to the other party.

2. What You Are Asking the Court to Do

Check every type of order you have decided to request. These selections are your own — I cannot suggest or recommend which to choose. If you're unsure what to request, that's a question for a licensed California attorney; once you've decided, I'll prepare the documents at your direction.

4. Supporting Facts — In Your Own Words

Your declaration to the court is your statement. What you write below goes into your declaration in your own words — I will not rewrite or summarize it. Write as if you're explaining to the judge. Include dates wherever you can.

6. Existing Court Orders

7. Supporting Documents

Check everything you have. After you submit this questionnaire, please email copies to newleafoffice.us@gmail.com — clear photos or scans are fine. Black out Social Security numbers and full account numbers first.

8. Serving the Other Party

After filing, the other party must be given copies of your Request for Order before the hearing. Depending on your case, this may need to be done by personal delivery by an adult who is not you.

9. Your Hearing

The court will set a hearing date when your Request for Order is filed. You attend and speak for yourself; I cannot appear or speak for you.

10. Final Review & Acknowledgments

Thank you! Your questionnaire was submitted. Please email your supporting documents to newleafoffice.us@gmail.com, and I'll follow up with next steps.
